During your first visit, we need to ask questions about your current and past health conditions. You can expedite the office visit by filling out a questionaire in advance. Each doctor may ask for information in a manner suitable for his practice style. Print the appropriate questionaire and complete it at your leisure. Then, bring it to the office with you.
We also ask you to provide details of medical insurance information on a separate form. We may make copies of insurance cards or documents to accurately record those long and unwieldy policy numbers used by insurance companies. Please view our financialpolicy page.
Forms and Instructions
Each surgeon has prepared a series of instructions to help you understand your surgery or procedure. At all times, you will be given appropriate instructions and explanations in person by your doctor. The same instructions are available on-line here and are provided as a convenience should you misplace those given to you by the doctor or nurse. From the rightmost column, select your doctor and view a list of available forms and instructions. Select the replacement you need and either view it on-line or print it.
